1. All evaluation components should be available on or before the first day of the fall semester for evaluations of the previous academic year. Since Faculty Reflection Forms cannot be submitted until after Student course evaluations are distributed to faculty, summer Faculty Reflection Forms and Student Course Evaluations may not be received until after this date.
  2. In addition to the required evaluation components submitted by faculty, the following information must also be collected:
        1. Class Grade Distributions – distributed by the Office of Faculty Evaluation
      1. Student Course Evaluations – distributed by the Office of Faculty Evaluation
      2. Student Complaints that have been Investigated and Substantiated, Faculty Discipline Information, and Information on Faculty Absences – maintained by each Department; the Department Chair is responsible for ensuring that any student complaints/faculty discipline issues have been appropriately addressed.
      3. List of Professional Development Hours and Lighthouse/Office Hours Compliance Data
      4. Previous Summary Evaluation Forms for all returning faculty, as well as any active Performance Improvement Plans from prior evaluations
      5. Other Departmental Required Information
  3. Department Chairs may choose to distribute materials for some portion of their faculty to designated reviewers (Assistant Department Chairs, Peer Reviewers, or Evaluation Committees, for example) to assist with the evaluation process. It is important that in this case, Designated Reviewers are well-versed on departmental/college rubrics and the rating schema.
  4. Department Chairs ensure that faculty who have not submitted the necessary evaluation materials by the deadline are contacted.
  5. Department Chairs must complete and approve Summary Evaluation Forms for every faculty member in their department. This evaluation might be based upon information from designated reviewers or from direct evaluation for each person based on the appropriate rubric, but the Department Chair is responsible for assigning ratings and the Overall Rating for each faculty member in their department, as well as making sure that every rated section has formative comments and that faculty who receive Approaching ACC Exemplary Teaching and Faculty Expectations or Does Not Meet ACC Faculty Expectations for any criteria are provided with a clear list of changes necessary to improve their performance in those areas. For faculty who have made improvements in response to comments in their previous evaluation or on a Performance Improvement Plan from the previous year, it is important that their progress towards or completion of those improvements be clearly noted.   6. Summary Evaluation Forms should then be sent to faculty for review, acceptance, and signatures when required, after which they are returned to the Department Chair. (Deans may optionally choose to review these forms before distribution to individual faculty or may choose to just specifically review forms for faculty receiving an Overall Rating of Does Not Meet ACC Faculty Expectations, but in general Deans are not required to be involved in the evaluation process until after forms have been returned to the Department Chair and any requested conferences with the Department Chair have been completed.)
  7. For any faculty who request a conference with the Department Chair or who receive a Does Not Meet Faculty Expectations as an overall rating, the following will occur:
        1. Schedule a conference between the faculty member and the Department Chair (initiated by the Department Chair). The Dean is given a list of the conference requests.
        2. The Department Chair conducts the conference and records any comments on the Supplemental Conference Form.
        3. If this conference is the result of a rating of Does Not Meet Faculty Expectations as noted above, the Department Chair and the faculty member must develop a
        4. Performance Improvement Plan with clearly defined outcomes and timeline. The Department will provide assistance in obtaining professional development required to make improvements.
        5. The Department Chair offers the faculty an opportunity to request a conference with the dean.
        6. If the faculty desires a conference with the dean, the Department Chair checks the appropriate box on the Supplemental Conference Form and then submits the Supplemental Conference Form, along with an updated Summary Evaluation Form (if appropriate) to the Dean
  8. All approved Summary Evaluation Forms will be returned to the Dean for final approval and distribution back to the faculty.
  9. All faculty evaluations for which no additional conferences are requested are expected to be completed, approved, and returned to faculty by the end of the fall semester.
