Annual Adjunct Faculty Evaluation Summary Form
For the faculty evaluation process to be truly meaningful for faculty, it is important that there be actionable, meaningful feedback. The purpose of the Annual Summary Evaluation Form is to provide faculty with the actionable, formative feedback necessary for faculty growth in addition to the summative ratings and comments.
IMPORTANT: We will not be submitting this form using a Word or pdf file this semester; this page is strictly for reference. We will be using a a web-app to submit Annual Faculty Evaluation Summary Forms. You can access the new Faculty Evaluation Summary System by logging in to MyACC and looking under “Faculty Development and Evaluation”.
Information on the new system is posted here. (An appendix with instructions will be added to this Manual in the future.)
