This Faculty Reflection Form <\/i>is meant to encourage self-reflection and to provide more complete information about what faculty have been working on. It should be submitted after every fall and spring semester (summer if required by your department), after you have received your student course evaluation results.<\/p>\n
These forms will be considered during your annual faculty evaluation to provide some insight into your Instructional Performance (questions 1 and 2), <\/i>your Student Feedback and\u00a0 Faculty Reflection (question 3), <\/i> your Professional Development and Growth (question 4), <\/i>and your Professional Service (question 5, required for full-time faculty only) <\/i>. The form encourages you to reflect on what happened this semester. It also allows you to include your perspective on your student feedback and can be used to inform your department about any non-instructional work you have been doing. Your responses here are not expected to be particularly lengthy; write only as much as is needed to answer the questions.<\/p>\n
