It should be submitted after every fall and spring semester (summer if required by your department), after you have received your student course evaluation results.<\/p>\n<p>These forms will be considered during your annual faculty evaluation to provide some insight into your <i>Instructional Performance (questions 1 and 2), <\/i>your <i>Student Feedback and\u00a0 Faculty Reflection (question 3), <\/i> your <i>Professional Development and Growth (question 4), <\/i>and your <i>Professional Service (question 5, required for full-time faculty only) <\/i>. The form encourages you to reflect on what happened this semester. It also allows you to include your perspective on your student feedback and can be used to inform your department about any non-instructional work you have been doing. Your responses here are not expected to be particularly lengthy; write only as much as is needed to answer the questions.<\/p>\n<p>Please note that:<\/p>\n<ul>\n<li style=\"list-style-type: none\">\n<ul>\n<li>Faculty teaching in a single department submit only one Faculty Reflection Form for each fall semester and each spring semester, covering all of the classes taught that semester<\/li>\n<li>Faculty teaching in multiple departments should submit one form to each department over the classes taught in that department<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><em>Please contact your department for information on how to submit this form (this varies by department).<\/em><\/p>\n","protected":false},"excerpt":{"rendered":"<p>This Faculty Reflection Form is meant to encourage self-reflection and to provide more complete information about what faculty have been working on.
