{"id":8405,"date":"2024-07-18T03:32:08","date_gmt":"2024-07-18T03:32:08","guid":{"rendered":"https:\/\/accinsweb8366.wpenginepowered.com\/evalhandbook\/?page_id=8405"},"modified":"2026-07-27T22:27:08","modified_gmt":"2026-07-27T22:27:08","slug":"evaluation-record-retention-policies","status":"publish","type":"page","link":"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/evaluation-record-retention-policies\/","title":{"rendered":"Evaluation Record Retention Policies"},"content":{"rendered":"<h3>Institutional<\/h3>\n<p>The retention of institutional records for faculty <a href=\"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/annual-summary-evaluation-form\/\"><em>Summary Evaluation<\/em>\u00a0<em>Forms <\/em><\/a>(<a href=\"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/appendix-3-annual-summary-evaluation-forms\/annual-adjunct-faculty-evaluation-summary-form\/\">Adjunct<\/a>\u00a0\/\u00a0<a href=\"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/appendix-3-annual-summary-evaluation-forms\/annual-full-time-faculty-evaluation-summary-form\/\">Full-Time<\/a>), student course evaluations, and other required documentation will follow the college record retention policies. Student course evaluations will be maintained by the <em>Faculty Evaluation Office.<\/em>\u00a0<em>Summary Evaluation Forms, <\/em>along with attached <em>Supplementary Conference Forms, <\/em>and<em> Performance Improvement Plans <\/em>will be maintained in the ACC <em><a href=\"https:\/\/faceval.apps.austincc.edu\/\">Faculty Evaluation Summary System app<\/a>\u00a0<\/em>as well as saved into each faculty member&#8217;s personnel file, as maintained by Human Resources.<\/p>\n<h3>Department<\/h3>\n<ul>\n<li style=\"list-style-type: none\">\n<ul>\n<li>All evaluation materials will be retained for at least three years for all faculty.<\/li>\n<li>Particular note should be made of faculty on a <a href=\"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/appendix-1-performance-improvement-plan\/\">Performance Improvement Plan<\/a> for reexamination in the following year.<\/li>\n<li><em><a href=\"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/annual-summary-evaluation-form\/\">Summary Evaluation\u00a0Forms <\/a> <\/em>(<a href=\"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/appendix-3-annual-summary-evaluation-forms\/annual-adjunct-faculty-evaluation-summary-form\/\">Adjunct<\/a>\u00a0\/\u00a0<a href=\"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/appendix-3-annual-summary-evaluation-forms\/annual-full-time-faculty-evaluation-summary-form\/\">Full-Time<\/a>), <em>Supplementary Conference Forms<\/em>, and <a href=\"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/appendix-1-performance-improvement-plan\/\"><em>Performance Improvement Plans<\/em><\/a> will be retained indefinitely for all active faculty (these will be included with evaluations material for future reviewers).<\/li>\n<li><em><a href=\"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/annual-summary-evaluation-form\/\">Summary Evaluation\u00a0Forms <\/a> <\/em>(<a href=\"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/appendix-3-annual-summary-evaluation-forms\/annual-adjunct-faculty-evaluation-summary-form\/\">Adjunct<\/a>\u00a0\/\u00a0<a href=\"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/appendix-3-annual-summary-evaluation-forms\/annual-full-time-faculty-evaluation-summary-form\/\">Full-Time<\/a>) for inactive faculty will be retained for at least six years.<\/li>\n<li><em>Summary Evaluation Forms<\/em> and any attached materials will be maintained in the ACC\u00a0<em><a href=\"https:\/\/faceval.apps.austincc.edu\/\">Faculty Evaluation Summary System app<\/a>\u00a0<\/em>as well as saved into each faculty member&#8217;s personnel file, as maintained by Human Resources.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h3>Dean<\/h3>\n<ul>\n<li style=\"list-style-type: none\">\n<ul>\n<li><a href=\"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/annual-summary-evaluation-form\/\"><em>Summary Evaluation<\/em>\u00a0<em>Forms <\/em><\/a>(<a href=\"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/appendix-3-annual-summary-evaluation-forms\/annual-adjunct-faculty-evaluation-summary-form\/\">Adjunct<\/a>\u00a0\/\u00a0<a href=\"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/appendix-3-annual-summary-evaluation-forms\/annual-full-time-faculty-evaluation-summary-form\/\">Full-Time<\/a>), <em>Supplementary Conference Forms<\/em>, and <a href=\"https:\/\/instruction.austincc.edu\/accfacultyevalmanual\/appendix-1-performance-improvement-plan\/\"><em>Performance Improvement Plans<\/em><\/a> will be retained indefinitely for all active faculty.<\/li>\n<li>Maintains evaluation materials for all Department Chairs (in their capacity as Department Chair) in their area.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h3>Faculty members<\/h3>\n<ul>\n<li style=\"list-style-type: none\">\n<ul>\n<li>Although departments are expected to maintain copies of all evaluation materials for at least three years, it is recommended that individual faculty members maintain copies of their own materials for at least that period of time as well.<\/li>\n<li>Please use this <a href=\"https:\/\/form.asana.com\/?k=R_BGhtDQrkR34H7xVS7zXA&amp;d=88303944259432\">Course Evaluation Request Form<\/a> to request your Course Evaluation reports for up to five years (faculty members and their direct supervisors only). All other requestors must submit an ACC <a href=\"https:\/\/offices.austincc.edu\/college-relations-and-marketing\/open-records-requests\/\">Open Records request<\/a>.<\/li>\n<li>To view\/print Faculty Evaluation Summary forms over academic year 2024-2025, faculty, department chairs and Deans can find them in the <a href=\"https:\/\/faceval.apps.austincc.edu\/\">Faculty Evaluation Summary System<\/a>. To request Faculty Evaluation Summary forms over earlier years, please <a href=\"https:\/\/acchelp.austincc.edu\/\">submit a TDX ticket<\/a>.
